Parental Consent Form for Registering of any In-person or Virtual JEI Programs

REGISTRATION FOR JEI ACADEMIC PROGRAMS

1. Registration is deemed complete and students are enrolled with JEI Learning Center once the registration form and all payment information are submitted and approved by the administration. In-person & Virtual Classes will not be held until registration is complete.

2. A one-time $75.00 registration fee per child is required upon enrollment.

3. A $50.00 diagnostic assessment fee per subject is required upon enrollment for new JEI students.
   A $40.00 reassessment fee per subject is applicable to returning JEI students who have been absent for more than 4 months.

4. The monthly tuition fee covers a 4-week program cycle and is collected as per the payment schedule agreement for services to be rendered in the following 4 classes. Once the student attends one class session; the monthly tuition fee is non-refundable.

5. The last month's payment must be submitted along with your 1st monthly payment. The last month's payment act as partial payment for your last registered month at JEI. In order to utilize the last month's payment, a written withdrawal notice must be submitted 4 classes prior to your last registered month at JEI. Failure to submit your withdrawal notices as required, the last month's payment will be forfeited. A re-submission of the last month's payment is required should you terminate your enrollment over 30 days.

6. All tuition payments returned to the office “Non-Sufficient Funds” will carry a $25.00 service charge paid to JEI Learning Center.

7. If a refund is authorized by the Director, the refund amount is based on the number of classes completed using the monthly tuition matrix minus a $150 admin charge.

8. All JEI Special Program fees and Early Bird Pricing are non-refundable. (P.A. Day Camp, Winter Camp, March Break Camp, Summer Camp, Summer Classes)

9. Tuition fees and subjects are non-transferable. Each student must be registered individually. **Monthly fees reflect annual fees that have been distributed evenly over the entire (ten) 10-month school year. Monthly program = 4 classes, 38 weeks = 38 classes ***Please refer to our JEI Calendar for the weeks of JEI classes. 
    

ATTENDANCE

1. Students are required to attend each class session.

2. JEI Classes that are regularly scheduled on days that fall on a statutory holiday are permitted to have the make-up class completed within the month. These include Labour Day, Thanksgiving Day, Family Day, Good Friday, Easter Monday and Victoria Day. This condition is not applicable to After School Club.

3. If the student is absent from class due to illness or other circumstances, it is the responsibility of the student and/or parents to obtain any missed homework or arrangements should be made to reschedule a class in advance. Classes must be rescheduled within the month and cannot carry over to other months. No refund is available for missed classes.

4. JEI must be notified 48 hours prior to a private class being cancelled. Classes that are cancelled with less than 5 hours notice are forfeited.

5. If the student is picked up late (15 minutes or more), there will be an additional $10.00 fee for every 15 minutes.
    

STUDENTS/PARENTS RESPONSIBILITIES

1. It is the responsibility of the parent(s) and/or guardian(s) to provide JEI Learning Center with any additional information that will assist with the child’s learning and success.

2. A $10.00 book deposit fee is required for the Reading & Writing Program.

PRIVACY POLICY
I understand that while attending JEI Learning Center, my child will participate in programs and activities offered by JEI Learning Center. I hereby give permission for my child to attend JEI Learning Center and to fully participate in all JEI Learning Center programs and activities.

Contact: JEI Learning Center reserves the right to contact me via email or any online services for any promotional activities or events regarding JEI, its programs or facilities. JEI Learning Center has my explicit permission to send me and my child or children information pertaining to their academia (homework, progress, etc.), scheduling matters, field trips, excursions or special events, through any form of social media (Remind, email, etc.).

Dismissal of Student: JEI Learning Center reserves the right to dismiss, in its sole discretion, any student whose behaviour is deemed unsatisfactory or detrimental to the best interests of JEI Learning Center, themselves, other students, and/or staff, in which case no refunds will be made.

JEI Learning Center collects information about your family to provide the best and most appropriate care for your child. Your information (including email address) is retained to enable us to communicate with you if we have new information that may be of interest to you.

JEI Learning Center will not sell or rent your family information to any person of the organization. Information is only given to those directly involved in providing a service to your child, or as required by law. All personal information is processed and stored in a secure location with access controls. 

I consent to have my child participate in Virtual JEI Classroom via Zoom sessions through an online platform. In order to reduce the risk of spreading COVID-19 and comply with government-recommended restrictions, I understand that JEI Learning Center has temporarily modified its program to provide educational services through an online platform. I understand that classes may be recorded for internal quality assurance purposes and will not be shared with outside parties.

MEDICAL WAIVER

I hereby give permission for medical attention to be administered to my child by staff in the event of a medical emergency. When I cannot be contacted, I hereby give my consent to have my child transported to a hospital emergency room and the hospital and medical staff have my authorization to provide any treatment, at my expense, that a physician deems necessary for the well-being of my child.

I hereby waive and release JEI Learning Center and its trustees, officers, teachers, employees, counsellors, volunteers, and agents from any and all present and future claims, costs, liabilities, expenses, or judgments, including attorney’s fees and court costs, resulting from any damage, loss, personal injury or illness to my child and/or damage to my child’s property arising from or out of my child’s attendance or enrollment in, or out of my child’s participation in activities at or offered by JEI Learning Center.

LIABILITIES
The parent(s) or guardian(s) shall be liable for any and all damages and injuries caused by the student.

ALL TERMS AND CONDITIONS MAY BE SUBJECT TO CHANGE WITHOUT NOTICE.

PA DAY / WINTER / MARCH BREAK CAMP FEE/REFUND POLICY

1. All camp tuition fees are due upon registration and before students attend camp. 

2. A late pick-up fee will be charged automatically with a pre-authorized credit card form. If extended hours were not paid for in advance, $10 will be charged commencing at 3:10PM, 4:10PM, 5:10PM, and 6:05PM.

3. If the student’s behaviour or attitude is disruptive or harmful to themselves or others, they may be asked to leave JEI Camp, and the tuition fee is non-refundable.

4. All camp fees are non-refundable and may only be transferred to another student at the Director's discretion. 
    

SUMMER CAMP FEE/REFUND POLICY

1. All camp tuition fees are due upon registration and before students attend camp. 

2. A late pick-up fee will be charged automatically with a pre-authorized credit card form. If extended hours were not paid for in advance, $10 will be charged commencing at 3:10PM, 4:10PM, 5:10PM, and 6:05PM.

3. Early bird pricing and Black Friday promotion fees are non-refundable, with no exceptions. Camp weeks are transferable (only within the registered Summer) and cannot be converted into credits for other summer programs or future use.

4. A 50% non-refundable administration fee applies to any and all cancellations before May 1, except medical withdrawals. No refunds will be given after May 1, 2025. All medical withdrawals require a medical certificate.

5. If the student’s behaviour or attitude is disruptive or harmful to themselves or others, they may be asked to leave JEI Camp, and the tuition fee is non-refundable.

6. I understand that homesickness, minor illness, change of family plans, dismissal from the program, and personal schedule conflicts/changes are not sufficient grounds for a refund.
